Tapering Protocol (CDC 2022)

  • Short-Term (<1yr): uses 2-Phase logic. Phase 1 is Linear (rate% of baseline dose) until 30% remains. Phase 2 is Exponential (rate% of current dose) to prevent withdrawal at low thresholds.
  • Long-Term (≥1yr): uses Slow Taper logic. Pure Exponential reduction (rate% of current dose) performed monthly.
  • Minimum decrement: drops <1 MME alternate Hold / Force (−2 MME) to keep the taper moving.
  • Jump-off: below 10 MME, extend dosing interval; below 5 MME, discontinue.
  • Organ gates: conversion factors adjust — or block entirely — for renal and hepatic impairment.
  • Dispensable rounding: drug doses snap to the nearest practical tablet increment (morphine 7.5 mg · oxycodone 2.5 mg · hydrocodone 2.5 mg · hydromorphone 2 mg · tramadol 25 mg), never below one increment until jump-off. Weeks that round to the same dose merge into a single step.
